- The receptionist will ask you about the following.
• Name, age, address, phone number (contact method), occupation
• Date and time when you can come for consultation
• Outline of consultation content

- If you are receiving treatment, consultation, or counseling at another medical institution or counseling institution, please apply after obtaining the consent of your doctor or interviewer.In some cases, we may ask for a letter of introduction.
- Depending on the content of your consultation, we may introduce you to other specialized institutions such as medical institutions.
- This center is not a medical institution and is not covered by health insurance.
- If you are coming by car, please let us know when you make a reservation.
